As the story is told by Patrolman David Slothower of Lower Pottsgrove Police Department, police received word of a disturbance occurring on Tanglewood Court in Lower Pottsgrove. While headed to the call, Slothower learned that Cook left the area at a high rate of speed in a 2008 red Lexus. The was informed, after Cook's plates were investigated, to keep an eye on his home on W. Ridge Pike.

Slothower arrived on scene and spoke with the female owner of the home. She showed the officer a large "picture window," which was broken. She stated she knew who threw the metal real estate sign through the window and identified Cook as the actor. The estimated cost to repair the window was determined to be $800.

There was a man sitting on the couch directly behind the window when it shattered. He allegedly had several cuts on his left hand from the glass. He stated he opened the front door and saw Cook and that he was "high and drunk." Cook started to yell for another man.

Apparently, the man Cook was yelling for is the father of a 4-year-old boy whose mother dates Cook. A child custody issue came up as the parties were arguing and Cook made a statement, according to the report, that he was going to kill the boy. A dog was then released from the house and chased Cook up toward Shire Drive. The man injured by the window caught up with Cook when he tripped and fell and thw two started to wrestle. During the struggle, Cook bit the man on his left forearm, causing an open wound which required medical treatment. Cook left the scene in his Lexus.

Limerick Sergeant Paul Marchese reported he went to Cook's address on W. Ridge Pike and his car was not at the location. While waiting in the driveway of the residence, at approximate 3:35 a.m., Cook's Lexus was seen approaching the house, slowed down and then began to speed up. Marchese followed the Lexus into the parking lot of a Wawa and approached the door. Cook was in the car with a bloody nose and mouth. Marchese asked Cook to step out of the vehicle, but his request was ignored. After several more attempts, Marchese opened the door and asked Cook to step out of the vehicle. This time he complied.

Marchese reported he noticed an unsteady gait form Cook, along with glassy eyes and a scent of alcoholic beverage on his person. Marchese asked Cook where he was coming from and he responded he was coming from his home. Marchese explained he knew that was not the case, but Cook insisted he was coming from his home. He also claimed he was bleeding because he walked into a door, and then changed his story, claiming he fell down.

Slothower asked Marchese to take Cook into custody for assault. Cook became uncooperative and was handcuffed. He was transported by Officer John Kehoe to Pottstown Memorial Medical Center for a blood withdraw after giving insufficient breath in the PBT test. Cook refused to submit to a blood withdraw and was subsequently handed over to Lower Pottsgrove Police.

Several attempts to reach the barber shop and Cook have failed, as the barber shop line goes straight to voicemail. The shop currently shows a "Closed" and "For Rent" sign on the property.
